Introduction

Welcome to Renew by CompassCare, a family medical practice and aesthetic clinic located in Ontario, Canada, offering both general medical services and cosmetic procedures, including Botox injections. By using our services, you agree to these Terms of Service ("ToS"). Please read them carefully. These ToS are governed by the laws of Ontario, Canada.

Medical Services

  1. Scope of Services: We provide comprehensive family medical care, including routine check-ups, diagnostics, and treatments. For Botox, we offer both cosmetic and therapeutic uses, such as for migraines or hyperhidrosis.

  2. Emergencies: For life-threatening emergencies, please visit the nearest emergency room.

  3. Patient History: Accurate medical history is crucial for safe treatment. Provide all relevant information, including allergies and medications.

  4. Service Refusal: We may refuse service if you withhold information, are non-compliant with policies, or are abusive.

Botox and Cosmetic Services

  1. Consultation: Required to discuss suitability and expectations. Results vary and are not guaranteed.

  2. Informed Consent: You must consent to treatment after understanding risks, benefits, and alternatives.

  3. Elective Nature: Botox for cosmetic purposes is elective; have realistic expectations.

  4. Medical vs. Cosmetic Use: Botox may be used for both medical conditions and cosmetic purposes, as determined by our professionals.

Appointments

  1. Scheduling: Available via phone.

  2. Cancellations/Rescheduling: Require 48 hours' notice to avoid fees.

  3. Late Arrivals: May result in rescheduling.

  4. No-Shows: Subject to fees and possible refusal of future appointments.

Fees and Payment

  1. Payment Responsibility: You are responsible for all fees, including those not covered by insurance.

  2. Insurance: Check coverage as Botox for cosmetic use is typically not covered.

  3. Accepted Payments: eTransfer and most major credit cards.

  4. Price Changes: Subject to change; we will inform you before procedures.

Confidentiality
Your information is protected under PHIPA. We share data only with authorized parties or with your consent, except as legally required. You may access your records; copying fees may apply.

Patient Responsibilities

  1. Follow Advice: Adhere to medical recommendations.

  2. Health Updates: Inform us of health changes.

  3. Respect Policies: Comply with clinic rules and behave respectfully.

Limitation of Liability
We are not liable for outcomes due to non-compliance with advice or Botox risks. Following medical advice is your responsibility.

Governing Law
These ToS are governed by Ontario laws.
